Notes to consider

YOU MUST BE ENROLLED IN THE PLAN FOR AT LEAST 6 MONTHS WITH AUTOPAY BEFORE YOU CAN CANCEL YOUR PET’S MEMBERSHIP. CANCELLING THE PLAN PRIOR TO 6 MONTHS HAS A PENALTY OF $200.

WELLNESS PLANS ARE ONLY AVAILABLE FOR DOGS 1 YEAR AND OLDER.

In order to enroll, we require an annual registration fee as well as a down payment.

Example: A client enrolls a pet in our Wellness Plan the Client will pay $30 registration plus plan's down payment.

Monthly payments follow immediately after enrollment. This will be an ongoing payment until cancellation.

If payments are not received, the plan is downgraded to unlimited office visits.

Unused services from any plan are not refundable. Registration fee is not refundable. You can cancel our plans only after 6 months of enrollment and past payments are not refundable.

With our plans, you get discounts on selected services but not products.

Basic care for dogs and cats with Down Payment $150 $25/month thereafter

Unlimited office visits
Vaccinations dog (DHLPP, Bordetella, Rabies 1 yr) cat (FVRCP, FelV, Rabies Purevax)- one per year
Intestinal parasite test (2 per year)
Anti-parasitic medication (2 per year) (Oral Strongid T suspension given in office)

Advanced care for dogs and cats with Down Payment $210 $35/month thereafter

Unlimited office visits
Vaccinations dog (DHLPP, Bordetella, Rabies 1 yr) cat (FVRCP, FelV, Rabies Purevax)
Heartworm Test- one per year
Intestinal parasite test (2 per year)
Anti-parasitic medication (2 per year) (Oral Strongid T suspension given in office)
Urinalysis (Chemical and microscopic analysis) one per year

North Hills Special for dogs and cats with Down Payment $270 $45/month thereafter

Unlimited office visits
Vaccinations dog (DHLPP, Bordetella, Rabies 1 yr) cat (FVRCP, FelV, Rabies Purevax)
Heartworm Test
Intestinal parasite test (2 per year)
Anti-parasitic medication (2 per year) (Oral Strongid T suspension given in office)
Dental cleaning ( One dental cleaning per year, dental extractions not included)
Urinalysis one per year
Blood Analysis: Complete Blood Count and Chemistry 15 parameters. One per year
Digital Xrays unlimited views (One time per year study)
Eye pressure test (one test per year)
Hospitalization: Two days of hospitalization, IV catheter, ICU hospitalization for 2 days, one 1000 ml Lactated Ringers or Sodium Chloride intravenous fluids and intravenous catheter placement. (2)

Additional Information:

Spay or neuter are NOT included on ANY of the above plans.

All surgeries include:

1. Recheck appointment 10-14 days after surgery (Spays and Neuters only).

2. Pain medication before, after surgery and pain medication to take home.

3. Intravenous catheter for anesthesia delivery and fluid administration. Also for emergency use.

4. Anesthesia and anesthesia monitoring every 2 minutes during the surgery.

Parameters monitored: Gas exchange (oxygen CO2), blood oxygenation, blood pressure with doppler, temperature, heart rate, respiration rate, ECG (electro cardiogram).

Surgeon uses cap, mask and sterile gown and gloves for the procedure.

Strongly recommended to perform complete blood-work prior to induction to anesthesia which is not included in the package. Blood analysis will give us crucial information about the patient's organ function.

Registration:

1. Registration fee $ 30 PLUS down payment

2. Monthly payments will be deducted directly from your credit/debit card until cancellation of plan

3. In order to receive services at North Hills Animal Hospital, patient's account must be current on payment. We reserve the right to deny services on overdue accounts. Declined cards will void plan.

4. To cancel wellness plan, simply give us a call and we will make arrangements. Registration fee and payments already made during the wellness plan enrollment are not refundable. If no services were rendered, we simply stop recurrent payments.

All services rendered during the wellness plan enrollment such as dentals, surgeries, laboratory diagnostics, x rays, hospitalization, etc must be paid in full at the time of cancellation of the plan.
